  1. You must be under the age of 13.
  2. Come in for your regular check-ups.
  3. If you don't have any cavities, you can sign up right away.
  4. If you do have a cavity, as soon as you get it filled you can sign up.
  5. Just fill out the Buddy Brush Club membership card.
